[Nine Clinic Hongdae] Hongdae Dermatology: Clear Up Dark Body Pigmentation Cleanly with Body Toning
A Hongdae dermatology clinic explains why body pigmentation on elbows, knees, and underarms is hard to fade with creams or scrubs, and how laser body toning targets melanin.

Hello there 🙂
I’m Dr. Goeun Lee, director of Nine Clinic Hongdae Dermatology.
Lately, more and more people have been coming to my clinic to prepare for summer.
One of the most common concerns among them is
pigmentation on areas such as the elbows, knees, and underarms.

“I wore a skirt, but when I looked at the photos, my knees looked so dark…”
“Why won’t my elbows get lighter no matter how hard I scrub them??”
Every time I hear comments like these,
I realize there are more people with similar worries than you might think.
If you came across this article while searching because of similar concerns,
it may be worth taking a few minutes to read through it.
Why is body pigmentation so hard to fade?
Rather than simply seeing body pigmentation as “skin that has gotten darker,”
it is more accurate to view it as the result of melanocytes deep in the skin being stimulated for a long time.
For example,
areas such as the elbows, knees, underarms, and groin are places where:
1. the skin folds repeatedly,
2. pressure and friction occur often,
3. and irritation builds up from hair removal, fragranced products, sweat, and more.
When this kind of irritation keeps happening,
the skin activates melanocytes to protect itself from the outside,
and as a result, pigmentation gradually develops over a wide area.
What makes this worse is that the melanin
often builds up not just on the surface of the skin but all the way down into the dermis.

In this situation,
simply applying whitening cream,
scrubbing,
or using a lotion that brightens temporarily
only makes the surface look briefly improved.
Since the melanin itself remains,
it tends to come back quickly, and fundamental change is difficult.
In my clinic, some people say,
“It looks lighter when I rub it with an exfoliating bath mitt,”
but that is closer to an illusion caused by only lightly sloughing off dead skin 🙂
Rather, it weakens the skin barrier further,
and pigment often settles even deeper,
so these home care routines can actually become a cause of a vicious cycle.
To sum up,
body pigmentation is often less about what is visible on the surface
and more about pigment that has built up in the skin over a long time.
Because simple home care has difficulty removing the melanin itself,
it is fair to say that it calls for a “pigment-targeting approach” using lasers.
How is simple whitening different from body toning at Hongdae Dermatology?

A practitioner in a white gown and protective glasses gives a laser treatment to a patient lying down.
Many people ask,
“So instead of whitening cream, should I just get whitening laser treatment?”
However, the “body toning” we recommend at Hongdae Dermatology
takes a different approach from simple “whitening” that just makes skin look whiter!
Whitening products
work through whitening ingredients that act on the surface of the skin (e.g., niacinamide, arbutin, and others)
to temporarily suppress melanin production,
or to make skin look brighter by smoothing out dead skin.
In other words, rather than “removing” pigment,
they are closer to “preventing” melanin from forming.
So they do not act on melanin that has already settled,
especially pigment that has sunk into the dermis,
and their effect on deeply set body pigmentation is inevitably limited.

The Spectra-VRM III dual pulsed Nd:YAG laser device, with its articulated arm and handpiece.
By contrast, body toning
uses picosecond-level high-speed energy or a 1064nm wavelength laser
to selectively break down the melanin pigment itself.
In other words, rather than simply making skin look brighter,
it precisely breaks down pigment
and encourages the body’s immune system to clear it out.
So even melanin that has already settled
gradually decreases with repeated laser exposure,
leading to a more even overall skin tone.
If this sounds like you, do seriously consider body toning!

Someone whose knee or elbow pigmentation really stands out in photos
→ Because it looks even darker against bright backgrounds or under strong lighting,
it often suddenly stands out in photos.
Someone who feels uneasy wearing sleeveless tops or swimwear because of underarm pigmentation
→ Many people feel it looks like a “shadow” rather than actual pigmentation,
but it may be tonal contrast caused by a buildup of pigment.
Someone who has tried body lotion, whitening cream, scrubs, and other methods without seeing any change
→ In these areas, the pigment is not just shallow,
as melanin often settles all the way into the dermis,
so home care alone may have its limits.
Someone who feels their skin has grown darker after hair removal, friction, or using scented products
→ Repeated irritation thins the skin barrier
and may lead to post-inflammatory hyperpigmentation (PIH),
which can be a stage that is hard to reverse with simple moisturizers or lotions.
I heard there’s something you should know before getting body toning at Hongdae Dermatology?
Body pigmentation is less about simply being dark
and much more about where the pigment sits within the skin.

Even for the same elbow or knee pigmentation,
in some cases it sits lightly on the surface,
while in others melanin has sunk deep into the layers below,
and because the way a laser works differs completely between these two cases,
simply firing at the same intensity or repeating the same approach
can make results slower than expected,
or in some cases, leave behind only irritation.
That’s why, before treatment, we first check how deep the pigment is,
and based on the results, it is important to first decide whether to use a 532nm or a 1064nm wavelength. Haha
Last but not least, I’d like to share something.
Skin really does differ from person to person.
Even if people look similar on the outside,
some are sensitive to irritation,
while others have pigment that has settled deep within the skin.
That’s why, even for the same area, I believe it matters more to adjust laser intensity, wavelength, and even treatment intervals
within the range of stimulation the skin can tolerate, rather than taking the same approach every time.

please feel free to ask anytime 🙂

Frequently Asked Questions
How does Hongdae body toning work?
Body toning is a procedure that uses picosecond-level high-speed energy or a 1064nm wavelength laser to selectively destroy melanin pigment. The broken-down pigment is encouraged to be cleared out by the body’s immune system, and with repeated exposure, it gradually decreases, leading to a more even overall skin tone.
Can whitening cream alone fade body pigmentation?
Whitening cream alone makes it hard to fundamentally improve body pigmentation that has already settled deeply. Whitening ingredients work by temporarily suppressing melanin production or smoothing dead skin to make skin look brighter, so their effect may be limited on pigment that has sunk into the dermis.
What should be checked before body toning?
Before treatment, it is important to first check how deep the pigment sits in the skin. Based on the results, the clinic decides whether to use a 532nm or 1064nm wavelength, and stresses that intensity, wavelength, and treatment intervals should be adjusted within the range the skin can tolerate.
Why don’t elbow and knee pigmentation fade easily?
These areas are subject to repeated folding, pressure and friction, hair removal, fragranced products, and sweat, which accumulate irritation and keep melanocytes active for long periods. The melanin often builds up not just on the skin’s surface but all the way into the dermis, so home care tends to see quick recurrence and makes fundamental change difficult.
Does scrubbing with an exfoliating bath mitt lighten pigmentation?
No. Looking lighter after rubbing with a bath mitt is closer to an illusion caused by lightly sloughing off dead skin. In fact, it often weakens the skin barrier and causes pigment to settle deeper, so this kind of home care can become a cause of a vicious cycle.
When is body toning worth considering?
Typical examples include cases where home care has produced no visible change, or where skin feels darker after hair removal, friction, or using scented products. It also mentions cases where knee or elbow pigmentation stands out in photos, or where underarm pigmentation makes sleeveless tops or swimwear feel uncomfortable.
