Hongdae Dermatology: Can Collagen Injections Help Hollow Under-Eye Dark Circles?
Discover why dark circles are often caused by under-eye hollowing rather than pigment, and how natural collagen stimulator injections offer a safer alternative to fillers.

Dark and dull under-eyes...
Could it be that they aren't actually caused by dark circles?!
Hello, this is Dr. Go-eun Lee from Nine Clinic in Hongdae 🙂
Among the patients visiting our clinic,
quite a few come in saying, "My dark circles have gotten so much worse."
However, when we look closely in person,
more often than not,
the true cause is volume hollows rather than actual pigmentation.
That is why starting with whitening treatments or toning lasers without identifying the exact cause
can cause you to miss the treatment you actually need 🥲
Even if they look similar on the surface,
pigmentation and hollows require completely different approaches.
So today,
I will explain in simple terms how to distinguish between the two,
and why I recommend collagen injections when hollows are the culprit!
Hongdae Dermatology: Not Dark Circles After All?!
A darkened under-eye area
is not always due to pigment deposition.

Along the inner lower eyelid,
there is an elongated groove known as the 'tear trough'.
This area marks the exact boundary where the orbital septum enclosing the eye meets
the underlying fat pad (malar/cheek fat pad).
The issue is that
as we age or as skin elasticity declines,
this fat can diminish or begin to sag downward,
causing this boundary line to look increasingly hollow and sunken.

When light hits that sunken hollow,
it casts a shadow,
which appears as a dark line from the outside—making people think, "I've developed dark circles."
Furthermore, under-eye skin is exceptionally thin,
making underlying muscles and blood vessels easily visible through it.
Therefore, when hollowing + shadows + visible vascular show-through overlap,
the area looks significantly more tired and dull!
If you assume this is pigmentation and repeatedly undergo whitening or pigment lasers,
it naturally yields no improvement,
and the excessive stimulation can even make the delicate skin more sensitive 🥲
That is why when someone visits me concerned about dark circles,
rather than simply looking at whether it looks dark,
I first evaluate three things:
the hollow's exact location, extent, and depth.
While it may seem straightforward from the outside,
individual differences are actually quite substantial.
Hongdae Dermatology: Fillers vs. Collagen Injections
When under-eyes are hollow,
many patients ask,
"Can't I just get fillers injected?"
In reality, however,
I do not casually recommend hyaluronic acid fillers for the under-eye area.
The reason is
that it is an area where unnatural results easily show.
There is also a significant risk of product clumping into visible nodules.
As explained earlier,
the skin under the eyes is very thin,
and depending on the angle of incoming light,
even a slight excess of artificial volume
can immediately look bumpy
or noticeably unnatural.

In addition, over time,
fillers may migrate downward or exhibit the 'Tyndall effect,'
where the filler scatters light, casting a bluish tint through thin skin.
Hence, rather than trying to fill under-eye hollows instantly,
I approach it with the philosophy that natural restoration is far superior,
and my treatment of choice for this
is collagen stimulator injections.

This stimulates the skin to regenerate its own collagen, replenishing volume naturally.
It can be thought of as a method that helps the skin restore itself gradually over time,
seamlessly
and without undue strain.
Hongdae Dermatology: When Are Collagen Injections Recommended?
① When your under-eyes constantly appear shadowed and look more sunken in the afternoon than in the morning
② When a tired, dull look persists despite whitening lasers or heavy concealer
③ When sunken under-eyes make your entire face appear fatigued all at once
For most people in these situations,
the primary cause is the shadow cast by deep volume depletion.

In particular, try checking the following signs:
⊙ Do your under-eyes look fine when smiling, but noticeably hollow with a neutral expression?
→ High likelihood of collagen volume deficit
⊙ When you gently create slight volume under the eye with your finger, does your expression brighten dramatically?
→ High likelihood of structural hollowing
Just because it appears dark does not mean it is true pigmentation dark circles.
An accurate assessment alone
can spare you from unnecessary, ineffective procedures ^^
Wrapping Up
Today, with the hope of helping those concerned about dark circles at our Hongdae dermatology clinic,
I shared insights on under-eye hollowing and collagen injections 🙂
While it might seem like a simple darkness issue from the surface,
in reality, 'hollowing' is very often the underlying cause.
Emphasizing once more how crucial it is to differentiate and approach this properly,
I will conclude today's post here.
Thank you very much for reading.

Frequently Asked Questions
Why don't whitening lasers work on my dark circles?
It is very likely that the darkness is caused by structural hollows and shadows rather than true pigment. When tear troughs deepen due to diminished skin elasticity or fat displacement, shadows are cast by incoming light, which laser whitening cannot resolve.
Why are fillers not commonly recommended for under-eye hollows?
Under-eye skin is exceptionally thin, making it vulnerable to nodule formation, visible lumps, or the Tyndall effect (a bluish tint from light scattering). Any excess or displaced volume can quickly look unnatural or uneven, requiring extreme caution.
How do under-eye collagen injections work to improve hollows?
Instead of instantly filling space with an artificial gel, collagen stimulators prompt the skin's fibroblasts to produce its own collagen over time. As autologous collagen gradually replenishes the tissue, under-eye hollows soften naturally and smoothly without stiffness.
Is there a way to self-diagnose whether my issue is under-eye hollowing?
Yes. If your under-eyes look sunken when your face is neutral but improve when smiling, or if supporting the area lightly with a finger drastically brightens your expression, structural hollowing is likely. Additionally, if the shadowing appears deeper as the day progresses into the afternoon, volume depletion is a strong possibility.
